Support that goes beyond appointments

Managing a long-term condition is about what happens between appointments too.

Our Population Health Programmes combine virtual and in-person healthcare to give you more continuous support. Your clinical team follows your progress over time, helping to spot changes earlier, review your treatment, and keep your health moving in the right direction.

Your programme is personalised around your condition, health goals, and the level of support you need.

Who our programmes support

Our programmes are available for selected people managing conditions and health risks including:

  • Type 2 diabetes
  • Prediabetes
  • Hypertension
  • Cardiovascular risk
  • Obesity
  • Metabolic syndrome
  • Selected mental health conditions

Is a programme right for you?

These programmes are designed for people who may benefit from closer support with an ongoing condition or health risk. Before you join, we’ll review your health needs, current treatment, the level of monitoring you need, and your insurance coverage.

If a programme is suitable, we’ll explain what it includes and what to expect.

How our programmes work

We understand your needs

Your health, current treatment, goals, level of risk, and eligibility are reviewed before your programme begins.

We build a plan around you

Your clinical team develops a personalised plan that may include monitoring, consultations, treatment reviews, and lifestyle support.

We follow your progress

You share agreed health readings, symptoms, or other information, and your team reviews how you’re doing over time.

We respond when something changes

If something needs attention, your team can follow up, review your treatment, and help coordinate the right next step.

What your programme may include

Regular clinical follow-up

Planned consultations with Mediclinic doctors and nurses, including virtual nursing rounds where appropriate, help your team review your progress and make changes when needed.

Treatment that evolves with you

Your care may include medication reviews, laboratory monitoring, and adjustments to your treatment plan as your needs change.

Specialist and lifestyle support

Where appropriate, your programme may include specialist input, health coaching, nutrition guidance, or exercise support.

Connected care across Mediclinic

If you need additional support, your team can help coordinate referrals, specialist appointments, laboratory tests, radiology, or a nurse home visit where clinically appropriate.

Medication and equipment support

Medication and medical equipment may be arranged for delivery, depending on your treatment plan and insurance arrangements.

Remote monitoring

Depending on your programme, the Huma app, medical-grade devices, or approved wearables may help your team track readings and symptoms, spot changes, and follow up when needed.

Population health programmes

Care, connected

Your care is continuously documented in your health record, helping the clinicians and services involved in your treatment stay connected.

Your programme follows evidence-based clinical pathways, with clear steps for escalating your care when needed. This continuity supports timely decisions and helps keep your care coordinated across Mediclinic Middle East.

Confidence and control

Over time, tracking your health can help you understand your usual readings, recognise changes, stay on track with your treatment plan, and feel more confident managing your condition day to day.

With guidance from your team on areas like medication, nutrition, and exercise where appropriate, you can take a more active role in your health. Together, this approach is designed to support better long-term control, reduce the risk of preventable complications, and help avoid unnecessary emergency visits.

Frequently asked questions

Coverage depends on your insurer, plan, and the support included in your programme. We’ll check this with you before you join, so you know what’s covered from the start.

There’s no set length of time. It will depend on your condition, how you’re progressing, and the level of support you need along the way.

Yes. Much of your programme may be managed remotely, wherever is convenient for you, with in-person appointments or services arranged when you need them.

Yes. Virtual consultations within the programme are delivered securely, so you can speak with your clinical team remotely when this forms part of your care.

Your team will explain what to track and how often. If you use the Huma app, it can remind you when readings or questionnaires are due, helping you stay on track with your programme.

You won’t be left to work it out on your own. Your team reviews the information you share and may get in touch if a reading needs attention or an expected reading is missed, so they can check how you’re doing and decide whether anything needs to change.

Yes. Your team can help coordinate appointments and follow-ups that form part of your programme, as well as connect you with other Mediclinic services when you need them.

Your programme is delivered by licensed Mediclinic Middle East clinicians and follows evidence-based guidance and relevant regulatory requirements. Clear clinical pathways are also in place so your care can be escalated when needed.

Just let your care team know, or call 800 2022 for help with the Huma app. They can help you understand what to record and support you with the app or any device that forms part of your programme.
